• All “Open” events are open to any entrant. All other events are subject to the Eligibility Requirements in the Attached Rules.
• Please note new rules regarding "Club Pros/Advanced Players"
• In all doubles events, each partner must submit a separate entry form and complete the seeding data at the end of the form

ENTRY FEES:

Entry Fee is $45 for each adult (age 18 and over) which includes a $30 tournament fee and $15 for your 2010 ECTA membership. If your 2010 dues have previously been paid, send an email advice of the same to ECTA Treasurer, This e-mail address is being protected from spambots. You need JavaScript enabled to view it , and any duplicate payment will be returned. For each junior (age 17 & under), the entry fee is a total of $15 with membership fees waived.

PAYMENT:

Payment may be made by ~ PayPal or by ~ Check, payable to Evanston Community Tennis Association and delivered to: ECTA Treasurer, 1313 Cleveland Street, Evanston, Illinois 60202. Check which payment method you wish. If you chose PayPal, click on the PayPal link below and complete your payment.

AGREEMENT TO ASSUME RISKS AND TO HOLD HARMLESS:

As a condition for entry, I, my family, and my heirs do hereby agree to assume all risks associated with my participation in this tournament and to hold Evanston Community Tennis Association, the Tournament Committee, the City of Evanston, Northwestern University, and the Evanston 4th of July Association, including their officers, boards of directors, committees, members, affiliates, and employees, jointly and severally harmless from any and all suits or claims resulting from any injury or accident that results from my participation in this tournament, regardless of cause.
